Required Forms

Students and their parents/guardians are required to read and fill out the following athletic forms before participation in a sport at a Davenport Community School District School.

Parent Acknowledgement Form

Iowa Code and the Davenport Community School District believe that all student-athletes should meet a set of criteria in order to participate in athletics and extra-curricular activities. Click on the link below to view the Academic Eligibility and Good Conduct Policy.  All athletes and parents/guardians must sign the following form acknowledging they understand the participation rules.

Physical Examination Form

An up-to-date physical is required for all athletes. A signed copy must be returned in order for an athlete to be eligible to participate in practices and games.
